Hi! Thank you for taking the time to fill out this intake form. These are a series of questions to begin the process of me seeking to understand who you are, how you are, and how we can work together through your postpartum journey. The intention is to allow you the space to consider your answers to these questions I’m asking, but definitely not for you to overthink things either. Provide as much or as little information & context for each question as you feel comfortable - there are no right or wrong answers.
